Yes, this unit will give you about 4 GPM of domestic hot water.
Yes. The specs say 180 gallons per hour of DHW (domestic hot water). If my math is correct (180 gallons divided by 60 minutes in an hour) leaves you with 3 gallons per minute . . . not 4. I may be missing something but that's just going off of their specs.

No, the unit does not have to be wall mounted, it is just an option offered.
Yes it should be wall mounted. I got the wall mount bracket and it just hangs on that. Very easy to mount. Just get another person to lift it on so you don't drop it. I don't see how you could use it any other way. It won't free stand because all the hot water comes out the bottom of the unit.
Sizing a boiler would depend on several factors, including your location, the insulation in your house, etc. As a general estimate, based on 45 BTU's per square foot, the PE-110 would be sufficient to heat your house.
According to the manufacturer, the output at 95% efficiency at 110,000 BTU input would be 99,000 BTU. The output at 78% efficiency at 110,000 BTU input would be 85,800 BTU.
