You should be able to get by with one 3/4" line for hot and one for cold.
That should work. We would recommend using a 1" x 1" x 1/2" copper tee.
Yes, you can use two of these inline with each other but they do not have the return. These manifolds are used mainly for plumbing applications so there is no return. All of our copper manifolds come with fittings on the end of them- we do not supply any bare ones.
The number of valve outlets depends on the number of runs you are making to this manifold. Most likely you will be running a hot and a cold from each area.
