Yes, supply adapters must be purchased separately. The kind that you need depends on the size of your supply tubing and whether you prefer threaded, crimp, or press connections. O-Rings are not replaceable.
You will need to purchase the proper MANABLOC supply adapter separately.
If you get one of the kits that has a 1" supply line specified and you're using pex for those lines, then you would just need the crimp rings. Since the manifold can accept either 3/4 or 1 inch supply lines, you need to get those yourself. Everything else you need at the manifold end is included.
It comes with the 18 1/2" compression fittings, but not the supply adapters.
Since you already have the crimping tools, we would probably recommend that you go with the brass PEX crimp manabloc. There are really no advantages to speak of between the two. You would not need any special tools if you opt to go with the compression MANABLOC.
It can be installed as you described; MANABLOCs will operate either vertically or horizontally.
It appears that you have is polybutylene pipe, not PEX pipe. We do not believe it will work

We recommend that you use the 1/2 inch. The volume of showers that you will be using will likely be bigger than what the 3/8 inch can produce.
You will need to consult local code: some places require a valve at the fixture and some do not.
The advertising is correct: Viega MANABLOCs may only be used for plumbing projects.
