Solved elevated flange.
Used this to support closet flange after removal of 3/4" mdf underlayment. Spacer splits so you can slide the halves under existing flange. Screw holes match those in flange. Tile guy can just tile around the flange and spacer, and not worry about trying to split backerboard and tile to slide under flange.
