Yes, there is a bracket from stamped galvanized sheet metal. It has to be first fastened to the fan housing. Use short screws or they will hit the fan inside. This gives some options how to mount the fan -- to keep access to the power box. The other side of the bracket will mount e.g. to a floor/ceiling between joists, as I did it. It "might" have come with two 4-inch hose/duct clamps as well. I am not sure if I bought them separately, though. I hope this helps.
generally the come with a mount, all the models I have purchased have had a mount. Look at the installation manual for the fan you choose, it will discuss mounting
Yes, it does. It's a large bracket that screws to the fan body with self-tapping screws, and there are holes on the other end of the bracket for mounting to wood.

Uncertain, but I have run our Fan continuously for days straight and I couldn't see an issue with doing it. It's a sweet little fan with noticeable output.
I don't know if it is "rated" for continuous use but that's how I've been using it. I have it on 8- 9 hours a day with no problems.
